• Referring appointment must be for a NEW patient to Newmarket Family Dental Care (NFDC).
• Referring appointment must be placed during August 2019, for attendance in either August 2019 or September 2019.
• Referring appointment must not be cancelled or postponed to be eligible for prize.
• Prize is non-transferrable for credit or cash
• If referring winner cancels appointment with NFDC after receiving prize, subscription prize will be halted.
• Online subscription commences Sept 1st and may be gifted to a friend or family member if not applicable to yourself.
• By entering this competition, an entrant is indicating his/her agreement to be bound by these terms and conditions.
